Caregivers of persons with developmental disabilities and persons with dementia are often concerned about the safety and well-being of their wards. The Developmental Disability Registry (DDR) Identity (ID) card was introduced with the aim of helping them integrate into society and live independently to the best of their ability. The information on the card will help members of the public identify and extend appropriate assistance to them, giving caregivers a better peace of mind.

There are two ways to apply:

  1. Scan and email all the requirements to ddr@bizlink.org.sg
  2. Send via Post or bring the documents to:

REQUIREMENTS: New Application

  • APPLICATION FORM duly filled out and signed (download application form)
  • DOCTOR’S MEMO with indication of the applicant’s disability
  • NRIC COPIES of the caregiver and the applicant
  • RECENT PHOTO of the applicant

REQUIREMENTS: Re-issue Card

  • EMAIL to Bizlink Centre’s DDR Team
  • PROVIDE applicant’s full name or NRIC
  • If details need to be UPDATED, kindly provide the information
  • ATTACH any relevant documents to update the details

APPLICATION PROCESS

  1. Upon receipt of the application, DDR Team will input the data in the NCSS Portal
  2. DDR Team will request for DDR Card to be printed. Please take note that the card is printed by another NCSS appointed vendor
  3. The NCSS appointed vendor sends the DDR card over via post to DDR Team
  4. DDR Team will mail out the card to applicant’s registered address
