Social Worker

Responsibilities:

Conducts needs assessment

  • Working with a multi-disciplinary team to conduct screening interview of clients/family for employment placement;
  • Participation in case conference;
  • Providing follow up actions in meeting clients’ needs;
  • Make home visits.

 Networking & Referrals

  • Liaise & network with agencies/VWOs relevant to the organization;
  • Provides feedback to agencies/VWOs when necessary;
  • Assists in processing referrals/application for any form of financial assistance or services.

Training & Vocational Rehabilitation

  • Assists in training of clients who are undergoing job trial & work preparation programmes;
  • Monitors clients’ progress and providing feedback to them and their families;
  • Provides counseling services for clients and family as and when necessary
  • Works with other professionals in the team to formulate and implement vocational rehabilitation for clients.

Project Activities

  • Participate in ad-hoc projects that enhance the overall vocational assessment effectiveness as well as increase the employability of clients.

Requirements:

  • Degree or Graduate Diploma in Social Work preferably with relevant experience.
  • Good communication skills and be able to relate with disabled people
  • Bilingual, preferably can speak in dialects
  • 5 days work week
