Responsibilities:
Conducts needs assessment
- Working with a multi-disciplinary team to conduct screening interview of clients/family for employment placement;
- Participation in case conference;
- Providing follow up actions in meeting clients’ needs;
- Make home visits.
Networking & Referrals
- Liaise & network with agencies/VWOs relevant to the organization;
- Provides feedback to agencies/VWOs when necessary;
- Assists in processing referrals/application for any form of financial assistance or services.
